Continuum of Colorado’s Home and Community Support Program is a licensed Home Care Agency established in 2012 providing quality services to adults and children by trustworthy and caring providers. Continuum of Colorado’s Home and Community Support Program is dedicated to collaborating with the individual and families in employing the best providers for individuals, regularly utilizing the Family Caregiver Act to hire family members and family referred applicants to provide a person-centered care plan which allows the individual to be an active participant. All employees complete required trainings specific to the individuals’ needs. We offer in-person services, as well as telehealth/video for certain approved services.
