Program Description

Alexander Home Healthcare serves individuals with acute and long term care needs, as well as the pediatric and adult population with disabilities in Larimer, Weld and Boulder counties.

Programs Available

In-Home Support Services (IHSS), Skilled Nursing, Family Care Education and Planning, Personal Care, Respite Care, and Homemaker Services. Other services include Alzheimer’s/ Dementia Care, Companion Care, coordinating home medical equipment, Patient/ Family Education, physical therapy, speech therapy, and help with hospital discharge/ transfers.
