We rely on research-based methods to develop programs for individuals to enhance their quality of life, independent living skills, communication, social and academic skills. Behavior and educational service delivery is intensive in nature and based on Applied Behavior Analysis principles designed around the individual's needs.
Children: At Aveanna Healthcare, medically fragile and special needs children receive the pediatric home care they need from an experienced professional in the familiar and loving environment of their own home. Adults: Aveanna Healthcare provides experienced, compassionate home care to disabled, injured, medically fragile, and elderly patients.
Axis has a team dedicated to engagement services. This team can connect with hard-to-reach populations. Through outreach programs such as Healthcare for the Homeless, Peer Recovery, Senior Reach, insurance Enrollment Services, and Jail-Based Behavioral Health Services, Axis team members engage with disadvantaged and un/underserved people in the community. These programs are designed to work closely with populations with high needs, chronic and acute issues, people experiencing addiction, homelessness, or isolation and help them find the supports they need in all aspects of their lives.
BAYADA Home Health Care is a trusted provider of clinical and assistive care services at home for adults of all ages.
Program provides a variety of health, social, and related support services—including nutrition services—for people with Alzheimer’s disease and related dementia, chronic illnesses, traumatic brain injuries, developmental disabilities, and other disabilities.
EXPAND's mission is to create a community of all abilities through recreation. We do this by supporting individuals in our community to participate in recreation and leisure programming. This includes providing specialized recreation and respite programs for individuals with disabilities as well as providing inclusion support in general Boulder Parks and Recreation Programs.
Breathe MBS is a certified PASA (Program Approved Service Agency) agency with a goal of helping those to aspire to, and achieve, a fulfilling life of new possibilities by providing support services to individuals with Developmental Disabilities (DD) and intellectual Developmental Disabilities (IDD). Our mission is to create a world of opportunity for all abilities. To advocate and assist individuals with intellectual and developmental disabilities to participate safely and successfully in the community.
The mission of Breckenridge Outdoor Education Center is to expand the potential of people of all abilities through meaningful, educational, and inspiring outdoor experiences.
We are a non-medical home care agency that offers traditional home care services in addition to our Brain-Body Wellness Program. Our goal is to improve our client's health and quality of life while maintaining their independence at home.
